Sebra is a dedicated Certified RN Care Manager who joined Nurse Advocate Partners in 2017 at our Massachusetts location. With a wealth of experience and expertise in various aspects of healthcare, Sebra has become an invaluable member of our team. Her strengths lie in case management, continuity of care, nursing navigation, and patient advocacy.
Throughout her career, Sebra has demonstrated a commitment to providing comprehensive and compassionate care across diverse clinical and community settings.
Her proficiency in navigating the complexities of healthcare systems ensures seamless continuity of care for her patients. Sebra is a passionate advocate for disease prevention and healthy living, extending her role beyond traditional healthcare boundaries. She firmly believes in the power of education and constantly seeks opportunities to expand her knowledge base. This commitment allows her to stay informed about the latest advancements in healthcare, benefiting the individuals under her care. Dedicated to patient advocacy, Sebra understands the importance of fostering strong patient-provider relationships. She works tirelessly to ensure that her clients receive the best possible care tailored to their unique needs. Sebra also serves as a community educator and facilitator for the Alzheimer's Association's care partners support group, having completed their Person-Centered Dementia Care Course.
In her spare time, Sebra has taken a media production class and recently launched a monthly podcast called The Mindful Aging Show. Beyond her professional commitments, she actively engages in community initiatives focused on health and wellness.
